Please try deactivate all plugins ( except plugins come from the theme ) and try it again. If it works, let's enable each plugin to check what's plugin caused this issue.
There is no any plugin can get your theme has 100 or close 100 on all mobile & desktop because the google speed on mobile based much on your hosting speed.

The setting works for categories. I'm guessing it's in the custom panel. It's easier to ask than crawl through and figure it out.

I'll do this another time. Content generation is more valueable I can always add an image at the top of the header at the mo.

No - you should solved it now.
Because you have a plugin conflict with lazyload images from the theme itself now. Let's solve it and the featured image will auto display on the top
If you add more an image there, it will be dupicate featured image when you solve the problems.
